Smoking cessation support program
The Health Insurance Association provides a subsidy for the cost of smoking cessation treatment under a doctor’s guidance.
We encourage you to take advantage of this program if you’re finding it hard to quit on your own.
Eligible persons
Insured persons aged 20 or older
Eligibility for examination
Support begins: On or after the date the insured person receiving support is granted eligibility by the Health Insurance Association.
Loss of eligibility: Note that you cannot apply for the subsidy if you lost your eligibility for coverage under the Health Insurance Association during the treatment period.

The subsidy
The Health Insurance Association will cover a portion of your copayment for the treatment.
(The subsidy will be paid only once for each individual and is capped at 10,000 yen. You are responsible for all costs beyond this amount.)
How to get examined
- STEP1The patient should make an appointment for treatment directly with the preferred medical care institution. (Make an appointment in time to submit the application no later than two weeks before the treatment date.)
- STEP2Obtain an Outpatient Smoking Cessation Treatment Application/Subsidy Application Form from your employer’s HR section. Fill out the necessary information and send the form to the Health Insurance Association via your employer’s HR section. Make sure the form arrives no later than two weeks before the treatment date.
*You must apply in advance to receive the subsidy. (If you apply after treatment starts, you will be responsible for the full cost of the treatment.) - STEP3After reviewing the Outpatient Smoking Cessation Treatment Application/Subsidy Application Form, the Association will send the form to your employer’s HR section with an approval number. Keep the returned form in a safe place.
- STEP4After starting treatment, submit the Outpatient Smoking Cessation Treatment Application/Subsidy Application Form to your employer’s HR section with the original receipts for five treatment visits attached.
- STEP5The Health Insurance Association will remit payment of the subsidy into the bank account in which the insured person receives salary payments via the employer’s HR section.
